Description

What happens when someone who has spent 22 years inside the healthcare system suddenly has to navigate it as a caregiver for the person she loves?

In this episode of Thought Atlas, I speak with Tiffany Auvil, a registered nurse, functional medicine health coach, and former healthcare leader who spent 22 years working within the same healthcare system. During that time, she worked in several roles, including LPN, population health nurse manager, family practice manager, and system director of outpatient clinics.

Then her husband was diagnosed with cancer.

Tiffany experienced healthcare from the other side of the hospital bed while caring for her husband through cancer, surgeries, treatment, complications, and ultimately his death in April 2025. Her experience revealed how administrative mistakes, communication gaps, insurance processes, and the complexity of the healthcare system can create enormous challenges for families already under immense emotional pressure.

That experience became the foundation for her work helping caregivers navigate what she calls the healthcare maze.

In this conversation, we explore:

• The difference between physical exhaustion and mental exhaustion• What caregiving taught Tiffany about burnout• Why caregivers may not recognize how mentally depleted they have become• The three major players Tiffany identifies within the healthcare system: providers, facilities, and insurance companies• Why those parts of the system can operate in silos• How administrative and coding errors can affect access to treatment• The insurance issue Tiffany encountered before her husband’s immunotherapy treatment• Why caregivers need to understand that they have more power than they may realize• Why a denied claim does not necessarily have to be the end of the conversation• The importance of understanding your own healthcare and advocating for yourself• Why providers may genuinely want to help while still being limited by the system around them• The role of hope during serious illness• Why optimism should exist alongside an honest understanding of reality• How Tiffany and her husband approached difficult treatment decisions together• The importance of allowing the patient to remain the decision-maker about their own care• How hospice became part of their support system• How Tiffany’s small West Virginia community helped her family financially during her husband’s treatment• What caregiving and loss taught Tiffany about love• Why preparing yourself for loss doesn’t necessarily prepare you for actually experiencing it• How Tiffany began taking care of herself after years of focusing on her husband• Why caregivers also need permission to prioritize their own wellbeing• And the message Tiffany wants caregivers to remember long after the caregiving journey ends

Her final word for caregivers is simple:

“Survive.”

Because sometimes, before you can think about what comes next, you have to make it through what is happening now.
