Two Western Australian mothers whose teenage daughters died by suicide have criticised the state government following an Auditor-General's report revealing that only one of 32 recommended mental health reforms has been implemented in four years. Hailee Hildebrandt, 18, took her life in January, while 13-year-old Kate Savage died six years ago after attempting suicide 10 times in two months. The Auditor-General's report highlights failures in planning, governance and leadership by the Mental Health Commission in supporting reforms to the child and adolescent mental health system.
