בשיעור רמב"ם היומי נתבארו כו"כ דינים בנוגע לעניני טומאה שגזרו חכמים בזמן בית שני, וביניהם ש"מי שהיו ידיו טהורות ונגע באחד מכתבי הקודש, נעשו ידיו שניות ומטמאין את התרומה כו'", וכן "רצועות התפילין עם התפילין" וכיו"ב. ובענין זה ישנו דבר תמוה לכאו' הדורש ביאור: מצינו שהכהנים הניחו תפילין (של ראש) בשעת העבודה, וכמו"כ מצינו שקראו בספר התורה במקדש! בשלמא בנוגע לנגיעה בתפילין וס"ת בשעה שנמצאים במקדש – הרי כבר כתב הרמב"ם שבמקדש לא גזרו שיטמאו הידים מנגיעה בכתבי-קודש ובתפילין. אבל אעפ"כ, מכיון שבפשטות הניחו תפילין גם מחוץ למקדש (ועד"ז בנוגע לכתבי-קודש), הרי נטמאו ידיהם מהנגיעה מחוץ למקדש! ול"קודש" צריכים הידים טבילה במקוה של מ' סאה, ולא מצינו שכל כהן שנכנס למקדש הי' חייב לטבול את ידיו?! ולפלא שלא אישתמיט שום תנא ואמורא, ראשונים ואחרונים, שידונו בזה בפרטיות!

Today’s Rambam discusses a number of Rabbinic decrees enacted in the times of the second Beis Hamikdash regarding impurity, including that “when one whose hands were pure touched one of the Holy Writings, his hands become shniyos, and impart impurity to terumah etc.” Similarly, “tefillin straps when connected to the tefillin” and the like. The question is: we find that the kohanim wore (head) tefillin during their service every day, and similarly, they read from a Sefer Torah in the Beis Hamikdash! True, with regards to touching tefillin and a Sefer Torah in the Mikdash, the Rambam has already told us that the decree doesn’t apply in the Mikdash. But the question still stands with regards to the fact that, like everyone else, kohanim wore their tefillin (and read from the Sefer Torah) outside the Mikdash, too. Seemingly, then, every kohen would have been obligated to immerse his hands in a kosher mikveh before entering the Mikdash – yet we don’t find such an obligation? And surprisingly, this issue is not discussed in detail in any Mishna or Gemara, nor in the Rishonim or Acharonim!
